近期，西安交通大学第一附属医院陈超教授、侯鹏教授及其团队合作在《Aggregate》（IF=18.8）上发表题为“Smart multi-functional aggregates reoxygenate tumor microenvironment through a two-pronged strategy to revitalize cancer immunotherapy”（智能多功能聚合体通过双管齐下的策略使肿瘤微环境重新充氧，以促进癌症免疫治疗）的研究。

PD-1/PD-L1抑制剂已成为晚期实体瘤的标准治疗方法；然而，总体反应率低与系统性副作用等挑战阻碍了其实施。缺氧促使肿瘤微环境的重塑是免疫疗法失败的主要原因。尽管目前已经报道了一些缓解缺氧的策略，但它们的个体局限性限制了进一步的改进。本文中，研究团队提出了一种新颖的双管齐下的策略，通过同时采用阿托伐醌（ATO，抑制氧消耗）和氧合血红蛋白（HbO2，直接补充氧气）在名为 NPs-aPD-1/HbO2/ATO的多功能聚合体中有效地解决缺氧问题。除了用这两种成分消除缺氧外，这种智能聚合体还包括白蛋白和 ROS 响应交联剂作为控释支架，以及用于免疫治疗的 PD-1 抗体 (aPD-1)。

有趣的是，NPs-aPD-1/HbO2/ATO 在体内表现出特殊的肿瘤靶向性，在肿瘤中的积累量比在肝脏中高约 4.2 倍。因此，这种聚合体不仅有效缓解缺氧并显著辅助aPD-1免疫治疗，而且同时解决了与每个成分单独给药相关的靶向和全身毒性问题。这项研究对药物靶向递送、解决肿瘤缺氧和推进免疫治疗具有重要的意义，为推进癌症治疗策略提供了有价值的见解。
